Please take a few minutes to fully complete our registration form so we may better serve you. We look forward to a long and rewarding relationship with you and your pet. Existing clients: use the same form to update your address, phone numbers, or other information.
- We encourage you to download our app, which connects you directly to our hospital, your pet's reminders, and any upcoming appointments. It is an easy way to communicate with us!
- Vaccine reminders will be sent through our app, by text, or by email. If these options are unavailable, a postcard will be mailed.
- Appointment reminders will be sent through our app, by text, or by email. If you cannot receive texts or emails, we will call you.
Downloadable forms
New clients are welcome to complete a patient form ahead of their first visit and bring it along, or email it to reception@oldwickanimalhospital.com.
Existing clients can also manage appointments, refills, and records through our PetDesk Pet Portal.
Need a hand getting here?
If you are in need of any assistance either for yourself or your pet, please let us know when making the appointment, or call us on your cell phone from the parking lot. If you or someone you know is physically unable to travel to this facility, it is the policy of Oldwick Animal Hospital that we will make a house call and/or provide transportation for that pet to the facility.
